Do You Have Cardiac Ischemia or Reperfusion Injury?
Ischemic Heart Disease
Ischemia-Reperfusion IRI is a risk following vascular or cardiovascular reperfusion procedures and post compartment syndrome fasciotomies. The severity depends on a combination of degree and duration of the occlusion and extent and type of tissue involved by the affected watershed area of capillary or arteriolar distribution.(1)
The following events occur with IRI:
- Tissue injury with tissue hypoxia and/or death from this initial ischemic insult occurs.
- Interrupted blood flow.
- Direct traumatic tissue injuries.
- Pressure induced injuries.
- Cold injuries or burns.
- Embolic, thrombotic, or localized inflammatory occlusion insults.
- Once circulation is restored, a secondary* recurrent ischemic effect may occur within the following 4 to 8 hours.
- Cell death can continue for up to 3 days.
- The release of endothelial chemotactic substances initiated by the original injury or insult creates an intravascular inflammatory response.
*This inflammatory response is at least partially responsible for further vascular occlusion of downstream tissues from edema. It is worsened by additional release from the second round of reactive oxygen species generated by the freshly oxygenated blood in an affected region that is depleted of protective free radical scavengers responding to the initial insult.
Hyperbaric oxygen therapy, if initiated early, has been found to ameliorate the damaging effects of reperfusion by:
- Early modulation of inflammation. (2)
- Maintenance of metabolic function in downstream tissues.(2)
- Reintroduction of oxidation scavengers.(3)
HBOT has potential clinical applications to treat ischemic pathologies.(4) The rationale and effects for using HBOT for reperfusion injury is demonstrated in a 1998 paper in the American Urological Journal , and in another study done by Departments of Internal Medicine, Undersea and Hyperbaric Medicine, Pathology, Nephrology, and Pediatrics, The Gulhane Military Medical Academy, Haydarpasa Training Hospital; Kadikoy in Istanbul, Turkey, demonstrated HBO showing marked improvement in tissue lesions’ healing
While HBOT has been studied after acute myocardial infarction (AMI) in conjunction with stenting/angioplasty and/or alteplase (tPA) administration, Eugene R. Worth, MD, M.Ed., FABA, ABPM/UHM, a board-certified anesthesiologist specialized in cardiac and vascular anesthesia, shares in this extensive article why he, as a doctor states the following:
“If I were scheduled to have an angioplasty/stenting or a CABG procedure, I would DEFINITELY approach the hyperbaric physician and cardiologist about having a standard wound healing HBOT treatment.

All the while, hyperbaric oxygen supports metabolic function through high concentration oxygen diffusion.

Thom offer this HBOT research because hyperoxia has effects of a number of cell signaling events that converge to influence cell recruitment/chemotaxis and gene regulation/ protein synthesis responses which mediate wound healing. Hyperbaric Oxygen Therapy; A Chosen Strategy for Late Radiation Tissue Injury in Gynecological Cancers

Gynecological cancers are the most common cancers affecting women today.
- Cervical
- Uterine

Did you know According to the American Cancer Society, there were an estimated 110,070 new cases diagnosed and approximately 32,120 deaths from gynecologic cancers in the U.S. in 2018.(1)
The cervix, and all it’s superior structures, are sterile.
This ultimately protects the uterine cavity and the upper genital tract by preventing bacterial invasion. This environment is maintained by the frequent shedding of the endometrium, thick cervical mucus and a narrow external os.(2)
The cervix is a hollow cylinder that connects the lower part of a woman’s uterus to her vagina.
Most cevical cancers begin in cells on the surface of the cervix placing a woman’s pelvic anatomy at risk.
Treating Post Radiation of Cervical Vaginal and Vulvar Cancer with Hyperbaric Oxygen Therapy
Many gynecologic cancers, (especially cervical, vaginal and vulvar), are treated with external-beam radiation therapy, and Hyperbaric Oxygen Therapy (HBOT).
The Journal of Urology further reports in a study by;
R. Mathews, N. Rajan, N. Josefson and others, Pelvic Radiation can lead to tissue hypoxia and poor healing. (3)
Strategies for managing pain and other clinical symptoms of post radiation have limited efficacy, however, HBOT may be an effective option for many pelvic patients.
According to Science Direct “a leading platform of peer reviewed literature” in Urology, Volume 65, Issue 4, clinical data reports positive responses when patients are treated with HBOT where there is tissue hypoxia and poor healing from radiation.
These positive rates range from 76% to 100%.
Additional reporting of a group after eleven years documents 74% of patients had no recurrence of symptoms.
This document also tells us that early (within 6 months) use of HBOT, post pelvic radiation has a greater therapeutic response rate to relieve symptoms of late radiation tissue injury (LRTI) and organ injury. (4)
In this extensive article written by authors; P. Craighead, M.A. Shea, J. Nation MD. Esmail, et al, they discuss pelvic radiation in women, the doses of radiation and the areas that can result in long term complications. The mechanics of LRTI are only partially understood. One major theory suggests that radiation causes progressive small blood vessel-stunting resulting in cellular hypoxia and damage which inhibits the ability of the irradiated tissue to repair itself, resulting in non-healing ulcers.
Their/this study showed positive therapeutic effects with HBOT for LRTI of the pelvis, and several other studies suggest that HBOT may be beneficial for improving quality of life in patients with radiation toxicity after treatment for pelvic malignancy.

In a 2008 study, authors, Clarke RE, Tenorio LM, Hussey JR, et al. from the Baromedical Research Foundation in Columbia, SC included long-term follow-up to evaluate the effectiveness of hyperbaric oxygen for pelvic radiation.In determining the effect of HBOT on radiation-induced soft tissue necrosis in patients who previously received treatment for a gynecologic malignancy, fourteen patients whose necrotic wounds failed to heal after 3 months of conservative therapy were enrolled in a prospective observational study. These women underwent 15 sessions of HBOT treatments.
All those with radiation necrosis of the vagina alone or in association with rectovaginal fistula had complete resolution of necrosis with hyperbaric oxygen. (9)
Furthermore, in the Department of Obstetrics and Gynecology, Oncology Division and Department of Radiotherapy in Jakarta Indonesia, a team of scientists; Sidik S, Hardjodisastro D, Setiabudy R, et al, evaluated quality of life resulting from HBOT after pelvic radiation. Their study concluded that HBOT decreased acute and late side effects, improving the quality of life of all their patients with pelvic radiation. (10)

Rapid Healing Rapid Recovery with HBOT
There are circumstances for various reasons that either a person must recover more quickly or that their recovery isn’t going as well as hoped.

Bladder Health Improved; Hyperbaric Oxygen Therapy

Many men and women suffer from post radiation treatment processes. Did you know that Hyperbaric Oxygen Therapy can Help?
Radiation cystitis is a recognized complication of pelvic radiotherapy. Damage from radiation treatment can either be acute (less than six months post radiation therapy) or delayed (more than six months maybe developing many years after treatment). This complication can have varying levels of irritation and functional impairment to the bladder mucosa.(1)
The Advances of Urology Newsletter states “Incidence of radiation cystitis ranges from 23% to 80% and severe cases range from 5% to 8%.” (2)
Mild symptoms: (These symptoms can resolve over time).
- Increased frequency and/or urgency to urinate, and possibly some dysuria.
- Infection showing microscopic hematuria. (see photo of hematuria below)
Chronic and detrimental symptoms:
(Late complications of pelvic radiation commonly encountered by urologists).
- Urinary incontinence.
- Gross hematuria.
- Progression of damage to the extent of fistula formation or necrotic bladder tissue.
Urology has treatment for both mild and chronic conditions which vary on the degree of symptoms.
Hyperbaric Oxygen Therapy Can Help!
Hyperbaric Oxygen Therapy (HBOT) is an effective treatment for radiation-induced hemorrhagic cystitis. According to Science Direct “a leading platform of peer reviewed literature” in Urology, Volume 65, Issue 4, clinical data reports positive response rates ranging from 76% to 100%. With additional reporting of a group after eleven years where 74% of patients had no recurrence of symptoms. (3)

HBOT, is a medical treatment prescribed by a physician (our Medical Director could consult with you to see if you are a good candidate for HBOT). During this consultation, our Medical Director will go over science backed protocol and contraindications.
HBOT is delivered in a special (medical grade hard metal) chamber where the patient will breathe in pure (medical grade 100%) oxygen in air pressure levels 1.5 to 3 times higher than average. The goal is to fill the blood with enough oxygen to repair tissues and restore normal body function.
In a hyperbaric oxygen environment, your lungs can gather more oxygen than would be possible breathing pure oxygen at normal air pressure.
Your blood carries this oxygen throughout your body. This helps fight bacteria and stimulate the release of substances called growth factors and stem cells, which promote healing.(4)
Overall, radiation cystitis can be detrimental to a patient’s wellbeing after already having gone through a great deal in regards to cancer treatment. Providers are becoming more aware of the drastic effects a dysfunctional bladder can have on overall quality of life.
Using HBOT, one clinical study in genitourinary medicine, observes early success “in the treatment of Fournier’s gangrene, radiation cystitis, and interstitial cystitis via the elimination of clinical symptoms such as pain.” (5)
The study further states; There are several beneficial properties of HBOT concomitant with elevated oxygen distribution in tissue including anti-inflammation, angiogenesis through vascular endothelial growth factor proliferation, augmented fibroblast activity through fibroblast growth factor proliferation, tissue and wound repair, enhancement of lymphocyte and macrophage activity, increased male testosterone secretion, and bactericidal activity.

HBOT is very important to stem cell biology; A Penn Study shows that after a course of treatments, there is an 8-fold increase in stem cell proliferation (6), which is crucial to injury repair. (7)

How Hyperbaric Oxygen Therapy Can Help in Cancer Treatment Processes
Sadly, the same cancer treatments that what will save a woman’s life may also have severe consequences for the rest of her life. From the analyzed cohort of 1945 women, 866 (45%) reported at least 1 toxicity that was severe/very severe, 9% reported unscheduled clinic visits for toxicity management, and 5% visited an emergency department or hospital. (1)

Hyperbaric oxygen therapy for late radiation tissue injury (LRTI)
Most Insurance companies will pay for post radiation injuries. (2)
A radiation injury or burn is damage to the skin or other tissue as an effect of radiation. (3)
This injury can be acute occurring within a few weeks of radiation, or can be chronic, producing varying degrees of damage to the skin and its underlying parts after a variable latent period of several months to several decades.
Hyperbaric oxygen therapy has been used as a treatment modality to promote repair of radiation-induced vascular changes.
In a trial test, authored by P. Craighead, MBChB., M.A.Shea-Budgell, MSc., J. Nation, MD., R. Esmail, BSC MSC., A.W. Evans, MD., M. parliament, MD., T.K. Oliver, MD. and N.A. Hagen, MD., 140 patients suffering from LRTI received HBOT.
They showed that 4 years post HBOT, their tissue oxygenation was tested using transcutaneous oxygen to reveal near-normal O2 levels. This implies angiogenesis is essentially permanent. (4)
Another research article states, “Hyperbaric oxygen therapy (HBOT) has been suggested as a treatment for LRTI based upon the ability to improve the blood supply to these tissues. It is postulated that HBOT may result in both healing of tissues and the prevention of problems following surgery. (5)
HBOT is both pain free and without side effects.The mechanism by which HBOT is thought to treat and repair radiation tissue injury is that it promotes radiation-induced vascular changes. Because of this, preoperative HBOT can contribute to a higher rate of surgical success.

There is a growing concern of post radiation induced lymphedema. Findings suggest that more than one in five women who survive breast cancer will develop arm lymphedema. (6)
Can Hyperbaric Oxygen Therapy Reduce Breast Cancer Treatment Related Lymphedema? A Pilot Study
This pilot study (7) authored by Jane Teas, Joan E. Cunningham, Lindsie Cone, Ken Jansen, Senthil K. Nitcheva, Dawen Xie, and William M. Butler was done with 10 healthy menopausal women between the ages of 58 and 62, suffering 9-18 years arm lymphedema. All 10 of these women were studied after breast cancer surgery and radiation. Seven of these women also had chemotherapy.
The results showed a reduction of 38% average hand lymphedema was observed at the end of HBO, independent of changes in body weight.
- HBOT reduces swelling while flooding the tissues with oxygen. The higher pressure in the chamber increases the amount of oxygen in the blood. HBOT aims to break the cycle of swelling, oxygen starvation, and tissue death.
- HBOT encourages the body’s oxygen radical scavengers to seek out the problem molecules and let healing continue.
- HBOT helps block the action of harmful bacteria and strengthens the body’s immune system. HBOT can disable toxins of certain bacteria. HBOT also increases oxygen concentration in the tissues further helping to resist infection. In addition, the therapy improves the ability of white blood cells to find and destroy invaders. (8)
HBOT can address this disabling and distressing disorder to reduce the individual and public health burden.
